Degenerative and skeletal changes associated with OA may be observed and staged radiographically. In the early stages of OA, osteophyte formation, joint space narrowing, subchondral sclerosis, and cysts may be visualized; soft tissue swelling is often absent. However, patients with joint symptoms do not always demonstrate radiographic OA, and radiologic severity of OA has not been shown to correlate with pain or functional status.

Radiographic changes, when present, are typically graded using the Kellgren and Lawrence grading system.

Studies have shown that injuries to the menisci and ACL are predisposing factors for the development of future knee OA, but they are not a consistent radiographic findings.
