Clinical relevance of cardiac MRI to HCM include:

  • Extent of fibrosis using late gadolinium enhancement, which is related to ventricular tachycardia and prognosis of HCM

  • Differentiation from other diseases associated with left ventricular hypertrophy

  • Risk stratification, especially the risk for sudden cardiac death

  • Choosing between surgical myectomy and alcohol septal ablation for treatment of LVOT obstruction

  • Diagnosis of HCM when echocardiogram is inconclusive

  • Perfusion MRI to determine myocardial blood flow and ischemia

  • Quantitative evaluation of LVOT obstruction, myocardial wall motion, and strain

  • HCM phenotyping
