The causes of obesity are multifaceted and include genetic, physiologic, behavioral, sociocultural, and environmental factors. Thus, an integrated and wide-ranging approach based on comprehensive lifestyle management (diet, exercise, and behavioral therapy), with consideration of pharmacologic therapy, is needed to achieve adequate weight loss in patients with obesity. Lifestyle management should include:

  • Self-monitoring of caloric intake and physical activity

  • Goal setting

  • Stimulus control

  • Nonfood rewards

  • Relapse prevention

Very low-calorie diets (< 800 cal/d) should be restricted to use in medical settings where medical supervision and high-intensity lifestyle intervention can be provided.
