Updated guidelines on the perioperative management of advanced ovarian cancer patients undergoing debulking surgery were published on August 18, 2021 by the European Society of Gynaecological Oncology (ESGO) in the International Journal of Gynecological Cancer.[1]

The ESGO guidelines provide recommendations on the following topics:

  • Timing of surgery in relation to therapy with targeted and antihormonal agents

  • Preoperative bowel preparation, skin antisepsis, and hair removal

  • Surgical safety checklists, patient positioning, and use of retractors

  • Prevention and management of upper abdominal complications

  • Prevention and management of infective and urologic complications

  • Management of bowel-related morbidity, prophylactic stoma formation, and stoma reversal

  • Antibiotic/microbiologic management and post-splenectomy management

  • Postoperative pain management

  • Perioperative thromboprophylaxis (pharmacologic and mechanical) and management of postoperative thromboembolic events

  • Wound considerations and complications

  • Nutritional management

  • Prehabilitation, enhanced recovery, and postoperative ileus prevention

  • Postoperative physiotherapy and mobilization

  • Frailty scores/management of the fragile patient

  • Psycho-oncologic and social support
