In earlier releases of the KDIGO guidelines, clinicians were advised to maintain phosphate in the normal range in patients with CKD. However, while acknowledging that preventing, rather than treating, hyperphosphatemia may be beneficial in patients with advanced CKD, the working group believes more data are needed to ascertain the risk/benefit ratio. Thus, current recommendations suggest phosphate-lowering therapies be administered to patients with progressive or persistent hyperphosphatemia only.
