HLA-DRB1*02/HLA-DRB1*03 alleles are not associated with a poor prognosis in patients with rheumatoid arthritis.

Predicting the course of an individual case of rheumatoid arthritis at the outset remains difficult, although the following all correlate with an unfavorable prognosis in terms of joint damage and disability:

  • HLA-DRB1*04/04 genotype

  • High serum titer of autoantibodies (eg, rheumatoid factor, anti–citrullinated protein antibodies)

  • Extra-articular manifestations

  • Large number of involved joints

  • Age younger than 30 years

  • Female sex

  • Systemic symptoms

  • Insidious onset
